I provide comprehensive independent private mental capacity assessments in South Yorkshire and surrounding areas for clients where there may be reason to doubt capacity or where decisions may be seen as contentious. I specialise in testamentary capacity for making or changing a will, taking out Lasting Powers of Attorney (Property & Financial Affairs and Health and Welfare) and completion of COP3 paperwork for deputyship orders. With over 16 years of experience working as a social worker with people living with dementia and other complex mental health needs, I have the knowledge and skills to carry out comprehensive capacity assessments and subsequent reports. I currently receive repeat referrals from several different solicitors across Doncaster, Rotherham, Barnsley and Sheffield.

I qualified as a social worker in 2009 gaining my BA Social Work Studies degree. I also have a post qualifying award from 2011 which enabled me to use the title of senior social worker. I have also completed a MA in Dementia Studies awarded in 2016. I am registered with Social Work England and I am a member of the British Association of Social Workers.

An assessment of capacity in the comfort of the clients own home to assess capacity for instructing a will/changing a will. Full report written up and returned to the solicitor within 1 week of assessment visit.
Capacity Assessment-COP3

An assessment of mental capacity for progression of deputyship orders. Includes face to face assessment and completion of COP3 paperwork. Paperwork completed and returned to solicitor within 1 week of the visit.
Capacity Assessment- LPA property and financial affairs OR health and welfa
£200
A Mental Capacity assessment for taking out a Lasting Power of Attorney for property and financial affairs OR Lasting Power of Attorney for health and welfare. Home visit to assess the client and full report returned to solicitor within 1 week. See below if both LPAs are required.

Oftentimes clients wish to take out both LPAs at the same time. This price includes Mental Capacity assessments for both LPA property and finances and LPA health and welfare on the same client at the same visit. Home visit and reports included.

This option includes three capacity assessments (on the same client at the same visit) .
1) Capacity assessment for LPA property and financial affairs
